Tongkat Ali and Testosterone: What the Trials Actually Measured

Does tongkat ali actually raise testosterone?

A 2022 systematic review and meta-analysis of five randomised trials reported significantly higher total testosterone with Eurycoma longifolia, with the effect confirmed in the hypogonadal subgroup. That is the strongest randomised signal any botanical in this category has produced. The pooled confidence interval is wide, the trials are small, and the doses used were 200 mg to 600 mg per day of a defined extract — far more than most blends disclose.

Tongkat ali is the one botanical in the male-vitality aisle that can point to a meta-analysis of randomised trials with a testosterone endpoint and a statistically significant result. That is an unusual position for a herbal ingredient, and it is still a smaller claim than the marketing around it suggests. The plant is Eurycoma longifolia, a shrub native to Malaysia and Indonesia whose root has long been used as a traditional tonic; the material studied in trials is a defined root extract rather than raw powdered root, and that distinction matters more than any other detail on a label.

What the meta-analysis of tongkat ali trials found

The best summary of the human evidence is a 2022 systematic review and meta-analysis in Medicina, which screened the literature to nine studies and pooled five randomised controlled trials. It reported that Eurycoma longifolia improved serum total testosterone in men across the pooled trials, with a standardised mean difference of 1.352 (95% CI 0.565 to 2.138, p = 0.001) under a random-effects model, and the effect was confirmed in the hypogonadal subgroup.

Two things are true of that result at once. It is a real pooled randomised signal, more than nearly any competing botanical has produced. And an interval running from 0.565 to 2.138 across five small trials is wide, which is why the authors wrote that more research is required before clinical use. The direction of effect is supported; the magnitude is not settled.

What doses and durations the individual trials used

Because tongkat ali is sold at wildly different strengths, the doses actually studied are the most useful thing on this page. A placebo-controlled, double-blind, matched-pair study gave 600 mg per day for two weeks to thirty-two healthy young men: the study of Eurycoma longifolia and the regulation of reproductive hormones in young males reported significant effects on testosterone, free testosterone and estradiol in favour of the treatment group, while luteinising hormone and follicle-stimulating hormone did not change — a pattern the authors read as a limited hypothalamic-pituitary-gonadal role in young men specifically.

At the other end of the age range, a six-month, double-blind, placebo-controlled randomised trial of Eurycoma longifolia and concurrent training on erectile function and testosterone levels ran for six months in forty-five men with androgen deficiency of the aging male, across four arms using a 200 mg supplement and three 60-minute training sessions a week. Erectile function improved with both interventions and improved most in the men who did both. The trial’s own stated limitation is the one that matters here: no arm tested the extract without training, so its separate contribution is hard to isolate.

A fourth study looked at stress rather than hormones directly. a trial of tongkat ali on stress hormones and psychological mood state in moderately stressed subjects supplemented sixty-three moderately stressed adults with a standardised hot-water root extract for four weeks and reported reduced cortisol exposure, improved salivary testosterone status and better scores for tension, anger and confusion. It is small, short and single-site, and it is the origin of most of the “tongkat ali for stress” positioning you will read elsewhere.

Why extract standardisation decides whether a product is comparable

Tongkat ali is not a single substance. Commercial products present it in at least three incompatible ways: raw root powder, a ratio extract labelled something like 100:1 or 200:1, and a hot-water extract standardised to a marker compound. Only the last is directly comparable to the material used above. Ratio claims are the weakest, because a ratio describes how much plant went into the vat, not how much of anything came out. That standardisation gap undermines much of the category: a systematic review and evidence-based analysis of ingredients in popular male testosterone and erectile dysfunction supplements and a review finding that testosterone-boosting supplement compositions and claims are not supported by the academic literature both found that ingredient labels frequently do not support the claims printed beside them.

What the evidence does not show

It does not show a large effect in men with normal testosterone. The meta-analytic signal was confirmed in the hypogonadal subgroup. Healthy men with mid-range levels are a different population, and a two-week study is too short to settle what happens to them over a year.

It does not show a clinical outcome. Higher serum testosterone is a laboratory number, and the trial that measured function measured it alongside exercise. The wider systematic review asking whether testosterone boosters really increase serum total testosterone is deliberately cautious about the whole category for this reason.

It does not show anything about milligram amounts inside a proprietary blend. A blend can contain a fraction of a studied dose and still list the ingredient truthfully — the arithmetic problem set out in how testosterone boosters hold up under audit.

A named ingredient is not a studied dose. Tongkat ali appears on many labels at amounts far below the 200 mg to 600 mg used in the trials described here. If the label does not disclose the amount, treat the research as background reading rather than as evidence about that product.

Safety, interactions and who should be careful

A forensic-toxicology review of the psychiatric and neurological adverse effects of herbal sexual enhancers covered Eurycoma longifolia among fifteen plants. It concluded that most appear safe at therapeutic doses, with problems concentrated in overdosing, self-medication with unstandardised products, and interactions when herbal preparations are combined with each other or with prescription drugs. The plant is not the hazard; the unlabelled product and the unmanaged stack are. How to judge a tongkat ali product in five checks

Check the extract description, not the plant name. Look for a root extract with a stated daily amount; a ratio alone tells you nothing comparable.

Check the daily amount against 200 mg. That is the lower of the two clearly reported doses on this page. Below it, you are outside the studied range.

Give it longer than a fortnight. The trial that reported functional change ran six months, and judging a botanical over ten days is the error explained in how long natural testosterone support takes.

Measure a baseline first. Men who supplement without ever testing have no way to know whether anything changed, and the symptoms that justify a test are listed in signs of low testosterone and when to test. If chronic stress is the real driver, the ground to cover before any extract is in cortisol, stress and libido.

Do not stack blindly. Tongkat ali is frequently combined with ingredients that have far weaker evidence, such as fadogia agrestis, or with materials carrying their own quality burden, such as shilajit. Adding names to a blend does not add evidence to it.

What the evidence shows, and what it does not

What it does not show: how large the effect is with any precision, whether it produces meaningful change in men whose testosterone is already normal, whether a given commercial extract matches the studied material, or that a few milligrams inside a proprietary blend does anything at all. Tongkat ali is the strongest botanical case in this category, which says as much about the category as about the plant.
